Our services are available to South Side residents ages 18 – 64 who are without health insurance or on Medicaid.

How It All Works

Our Model

Thrive South Side focuses on Medicaid and uninsured patients across 15 zip codes on the South Side, a demographic historically left behind by the health care system. Through our health navigation services, Thrive South Side works with patients to connect them with the care and resources they need.

A Community Health Worker (CHW) meets the patient at an event, clinic, within the community, or via partner referral.
The CHW conducts an intake interview to understand both medical and social needs, such as housing, food, and transportation.
The Nurse Care Coordinator creates a care plan and connects the patient to relevant providers and services.
Ongoing check-ins ensure patients follow through on appointments and stay supported.

Primary & Preventive Care

We ensure patients are connected to a trusted primary care physician — the first and most important step toward lasting wellness.

Maternal Health

We connect pregnant and postpartum mothers with comprehensive support, including high-risk care, ultrasounds, doula services, and nutrition counseling.

Heart Health

We offer monitoring systems for
high-risk patients after blood pressure screenings, as well as individual and group education about heart healthy issues.

Chronic Disease

We make living with diabetes, hypertension, asthma, or another chronic condition easier to manage.

Because 70% of a person’s health is shaped by life circumstances, SSHCO connects patients to essential resources including housing, food, workforce training, transportation, and more.
